4. Account Owner's Name If you're the only person on the account, this would be you.

Here's what you will need to transfer your mobile phone number from Verizon:
1. Account Number Your account number is usually at the top of any bill/invoice, or in your online account.
2. Number Transfer PIN If you have a standard account, the first step is to request a Number Transfer PIN. In order to transfer your Verizon number to another carrier, you will need to provide this PIN and your Verizon Account Number as authorization for the transfer.
How to request a Number Transfer PIN To request a Number Transfer PIN you must have a standard account and be the Account Owner or Account Manager
Option 1: Dial #PORT to request a Number Transfer PIN To request your Number Transfer PIN, dial #PORT from your smartphone. If the My Verizon app is installed on your device, you'll be directed to the Number Transfer PIN screen in the app. If the app is not installed, you'll be directed to the Number Transfer PIN screen on your mobile browser
Option 2: Request a Number Transfer PIN online You can also request a Number Transfer PIN online in My Verizon. To request a Number Transfer PIN online, go to the Number Transfer PIN page and click or tap Generate PIN. Your PIN and Account Number will display on the screen.
The Number Transfer PIN is valid for any line being transferred from your Verizon account. The Number Transfer PIN is valid for 7 days so it is important that you provide this information immediately to prevent fall out.
